If you’ve ever wondered why diabetes and fatty liver disease often show up together, you’re not alone


Across the globe, millions of people living with type 2 diabetes also have fatty liver disease (officially called MASLD or NAFLD). These two conditions are closely tied to insulin resistance, obesity, and poor metabolic health—making them a growing global health concern.
Here’s the deal: over 70% of people with type 2 diabetes also develop fatty liver disease, a CDC report says. The connection runs deep—your blood sugar, cholesterol, body fat, and liver health are all part of one system. And when one goes off balance, the others follow.

Both diabetes mellitus type 2 and fatty liver are closely connected


The underlying mechanism in both of the conditions is insulin resistance. There are different studies which have looked into the prevalence of fatty liver in people with diabetes and the prevalence is found to be as high as 70% in people with type 2 diabetes, says Dr. Anshu Alok, Senior Consultant- Endocrinology & Diabetes, Max Smart Super Specialty Hospital, Saket.
In diabetes, as the sugars are increased, it predisposes to increased fat storage in the liver causing fatty liver. Increased fat in the liver alters the metabolism in the liver and increased free fatty acids are released in the blood which is again related with worsening of the insulin resistance and worsening of the diabetes and its related complications. So together both these conditions form a vicious cycle and both are worsened because of these mechanisms.

How to control these conditions?


To control both these conditions, similar lifestyle changes are needed. A good low carbohydrate diet which includes a healthy source of protein, increased fiber, fresh vegetables, and fresh fruits are known to help both in diabetes and in fatty liver. Daily exercise close to 45-50 minutes of moderate intensity exercise is also known to help both with diabetes and with fatty liver, says Dr. Anshu Alok.
In people with obesity or overweight individuals, with these lifestyle changes, if they lose weight, it will help both diabetes and fatty liver, he adds.

Eat smart: The best diet for diabetes and fatty liver


When it comes to managing fatty liver and diabetes naturally, diet is everything. Choose foods that reduce inflammation and support your liver’s ability to process fat.
What to eat more of:
Colorful vegetables (aim for 5 servings a day)
Whole grains (brown rice, oats, quinoa)
Fruits (berries, apples, citrus)
Plant-based proteins (beans, lentils, tofu, nuts)
What to limit or avoid:Processed foods and fast foodSugary drinks and dessertsRed and processed meatsAlcohol (especially beer and spirits)
The Mediterranean diet and DASH diet are globally recognized for improving cholesterol, blood sugar, and liver fat. These eating plans are rich in healthy fats (like olive oil and nuts), fiber, and lean protein — perfect for protecting both your metabolism and liver health.

Move more: Exercise that protects your liver and blood sugar


You don’t need to hit the gym hard to make a difference. Regular physical activity improves insulin sensitivity and helps burn liver fat.
Aim for:
150 minutes of moderate exercise per week — brisk walking, swimming, cycling, or dancing.
Add 2 sessions of light strength training weekly for extra metabolic benefits.
Even small steps like walking after meals or taking stairs can help. The secret is consistency — doing something every day that keeps your body moving.
