International women's day: Common gynaecological problems that women deal with

Women health conditions

Many conditions can all have significant impacts on women's health, particularly due to the societal inhibitions and taboos surrounding these conditions. Women may feel embarrassed or ashamed to seek medical help for issues related to their anal, reproductive or pelvic health, leading to delays in diagnosis and treatment and exacerbation of the issue.

Oftentimes people may confuse these three conditions with each other, so it is essential to have clarity over each one of them.

2/6

Fissure

A fissure is essentially a tear in the anal canal typically caused by constipation or passing hard stool. As stool passes through the anus, it can tear the lining, leading to bleeding and intense pain due to muscle spasms. This pain can be debilitating, causing a person to avoid bowel movements, which exacerbates constipation and perpetuates a vicious cycle of tearing and pain. Remissions may occur, but lifestyle factors often trigger recurrences. It is crucial to consult a doctor early on when symptoms such as boils near the anus, pain, fever, constipation, and urinary retention arise.

PCOS, or polycystic ovarian syndrome

PCOS affects the hormonal balance and reproductive health of Indian women and is a common illness. A balanced diet, frequent exercise, and stress reduction are just a few examples of lifestyle changes that can be quite helpful in controlling PCOS symptoms. Women with PCOS must check their hormonal health and seek advice from medical professionals for individualized treatment strategies.

Piles

According to Dr. Udipta Ray, Director- GI, General Surgery and Robot-assisted Surgery- Fortis Hospital, Anandapur, “If left untreated, fissures can progress, resulting in the development of sentinel piles, where a fleshy mass protrudes from the anus, further complicating symptoms. Women, particularly during pregnancy and after childbirth due to hormonal changes like increased progesterone, are more prone to fissures. However, men are also affected, albeit less frequently. It's crucial to note that bleeding or pain from the anus should never be considered normal and warrants medical attention to rule out more serious conditions like anal malignancy."

While some may explore alternative treatments, acute anal fissures typically respond well to medication, although surgery offers immediate relief and early cure. However, long-term management focuses on lifestyle adjustments such as staying hydrated, consuming fiber-rich foods, avoiding constipation, regular exercise, adequate sleep, and stress management. Newer medications combined with lifestyle modifications can effectively manage early-stage fissures. However, once the condition becomes chronic and sentinel piles develop, surgery may be necessary. Fortunately, advancements in surgical techniques, such as laser surgery, offer less invasive options with predictable results and minimal pain.

Breast Cancer

Breast cancer is a serious health problem that is becoming more common in Indian women. Mammograms, clinical tests, and routine breast self-examinations are crucial for early detection. Women should take the effort to learn about their family's medical history, make sure they get regular checkups, and take part in campaigns to raise awareness of breast cancer. Breast cancer prevention can also be aided by lifestyle choices including reducing alcohol intake and keeping a healthy weight.

6/6

Fistula

Fistula is a condition characterised by an infection originating from glands within the anus, which normally secrete a lubricating fluid to aid in the passage of stool. Factors like constipation can lead to infection of these glands, resulting in the formation of a pus-filled pocket. As the infection progresses, it can create an intensely painful collection of pus that tracks down and forms an external opening near the anus. It eventually bursts open to pour out pus and blood as a boil near the anus. However, this external opening does not signify the end of the issue, as the internal source of infection persists, leading to recurrent discharge of pus, pain, and itching.
