Understanding stage 0 cancer: The silent beginning of cancer

What does "Stage 0 cancer" actually mean?

Stage 0 cancer, also known as carcinoma in situ (CIS), refers to abnormal cells that are found only in the layer of cells where they first began to develop.
Think of it this way: it’s like finding a weed seed in the soil before it has had a chance to sprout and grow roots. The problem is contained in one spot and has not spread into any surrounding healthy tissue. It is non-invasive.
(Dr Nikhil Suresh Ghadyalpatil, Director - Medical Oncology, Apollo Cancer Centres, Hyderabad)

2/8

If it's "Stage 0," is it really cancer?

Yes, it is considered the earliest, non-invasive form of cancer. The cells look like cancer cells under a microscope and have the potential to grow and spread if left untreated. That's why your medical team takes it seriously and recommends a plan of action.

3/8

Will I feel any symptoms, like a lump or pain?

Almost never. The vast majority of Stage 0 cancers are asymptomatic. This is a critical point: you can't rely on your body to give you a warning sign. This is precisely why routine health screenings are so important.

4/8

So, how is it found if there are no symptoms?

Stage 0 cancer is most often discovered during routine screening tests. These are your first line of defense. Key examples include:
Mammograms for Ductal Carcinoma in Situ (DCIS) in the breast.
Pap tests for cervical abnormalities.
Colonoscopies for pre-cancerous polyps in the colon.
Skin exams for early, non-invasive skin cancers.

5/8

What happens after a screening test finds something?

If a screening test shows a suspicious area, the next step is a biopsy. A doctor will take a very small sample of the tissue for a pathologist to examine under a microscope. This examination is the only way to confirm a diagnosis of Stage 0 cancer.

6/8

Does this diagnosis mean I will definitely get invasive cancer?

Not necessarily. Not all Stage 0 lesions will progress to become invasive cancer. However, because it's impossible to know for sure which ones will and which ones won't, the safest approach is to treat them. Think of it as correcting a small course deviation on a map; a tiny, early adjustment prevents you from ending up in a very different and more difficult place later.

7/8

What is the treatment for Stage 0 cancer?

The main goal is to remove the abnormal cells completely. Treatment is highly effective and often less aggressive than for later-stage cancers. Options depend on the location and type, but commonly include:
Surgery: Often a local procedure to remove the abnormal tissue and a small margin of healthy tissue around it.
Radiation Therapy: Sometimes used after surgery to eliminate any remaining cells.
Hormone Therapy: May be used for certain types, like hormone-sensitive DCIS.
Active Surveillance: In some very low-risk cases, your doctor may recommend a "watchful waiting" approach with frequent monitoring.

8/8

What is the outlook after a Stage 0 diagnosis?

The prognosis is excellent. Because the cancer is non-invasive and contained, treatment is highly likely to result in a complete cure. This is the most treatable stage of cancer, which underscores the profound value of catching it early through regular check-ups.
