​​Vaginal cancer is rare but complex​



Vaginal cancer, a rare and often overlooked form of cancer, emerges in the cells lining the vagina—an essential organ connecting the cervix to the vulva. Unlike cancers originating in other parts of the body that occasionally spread to the vagina, vaginal cancer germinates within the vaginal walls, accounting for a mere 1 to 2% of gynaecological cancers.

Typically diagnosed as squamous cell carcinoma, this condition affects approximately 1 in 100,000 women. The prognosis significantly hinges on the stage of diagnosis; when confined to the vagina, the probability of successful treatment remains high. However, once it extends beyond the vaginal boundaries, combating the disease becomes significantly challenging.

​​Early symptoms of vaginal cancer​



Vaginal cancer often begins subtly, its initial symptoms easily mistaken for common ailments. As it progresses, distinctive signs emerge, prompting medical attention. Irregular vaginal bleeding, post-menopause or after intercourse, is a key indicator. Unusual discharge, along with a vaginal lump, hints at malignancy. Painful urination, frequent urges, constipation, and persistent pelvic discomfort could signify vaginal cancer and should not be ignored.

​​What causes vaginal cancer?​



According to Dr. Vinay Bhatia, Head, Molecular Biology National Reference Lab, Oncquest Laboratories Limited, Gurugram, “The genesis of vaginal cancer lies in the DNA alterations within vaginal cells. Healthy cells receive instructions from their DNA to grow and multiply at a controlled rate and die at a predetermined time. However, cancer cells deviate from this norm, rapidly multiplying and escaping the natural cell death process. These rogue cells can form a tumour, invading and damaging healthy tissues. Over time, cancer cells can metastasize, spreading to other body parts and wreaking havoc.”

While the precise cause of these DNA mutations remains elusive, several risk factors, such as smoking, human papillomavirus (HPV) infection, and a history of cervical cancer, increase the likelihood of developing vaginal cancer.

​​Diagnosis of vaginal cancer​



A timely and accurate diagnosis of vaginal cancer is crucial for devising an effective treatment plan. Healthcare professionals employ various tests and procedures to confirm the presence of cancer and determine its stage. Pelvic exams, utilizing a speculum and magnifying instruments, aid in inspecting the vagina and cervix for abnormalities. Colposcopy, a specialized examination with a lighted magnifying instrument, assists in magnifying the vaginal surface to identify potential cancerous changes. Biopsies, where tissue samples are collected and sent for testing, provide definitive confirmation of cancer.

​​Treatment of vaginal cancer​



Imaging tests such as X-rays, CT scans, MRI, and PET scans offer detailed insights into the cancer's extent and help in staging the disease. Additionally, procedures like cystoscopy and proctoscopy, employing tiny cameras, aid in assessing the cancer's spread to nearby organs. Based on the gathered information, the cancer is assigned a stage ranging from 1 to 4, signifying its severity and progression.

Treatment for vaginal cancer involves surgery, radiation therapy, chemotherapy, and targeted therapy, tailored to the cancer's stage, size, and location. Surgery removes tumours, radiation uses high-energy beams and chemotherapy/targeted therapy employs drugs. Combining these treatments is often the most effective, underscoring the need for personalized approaches against this rare yet formidable disease.
