What is Lyme disease? Scientists uncover an enzyme that could revolutionize the treatment of the disease
Lyme disease’s Achilles’ Heel has been discovered.
What does the study say?
Scientists have identified an enzyme that could be a promising target for developing new treatments for Lyme disease and potentially other tick-borne illnesses. Their findings, published recently in mBio, a journal of the American Society for Microbiology, could open the door to more effective therapies.
In this new study, researchers explored the function of BbLDH in B. burgdorferi and its potential as a therapeutic target. Using genetic, biochemical, and structural analysis, including X-ray crystallography, they identified BbLDH’s essential role in the bacterium’s growth and ability to infect a host.
Loss-of-function studies confirmed that BbLDH is necessary for the bacterium to thrive both in lab cultures and in living organisms. Additionally, the team performed high-throughput screening and discovered several promising LDH inhibitors that could serve as the basis for future treatments.
Corresponding study author Chunhao (Chris) Li, M.S., M.D., Edward Myers Endowed Professor, the Philips Research Institute for Oral Health, School of Dentistry, Virginia Commonwealth University, said, “We discovered that BbLDH has a unique biochemical and structural feature and it is essential for B. burgdorferi growth and infectivity. BbLDH can serve as an ideal target for developing genus-specific inhibitors that can be potentially used to treat and prevent Lyme disease.” Li also added, “This report also sheds new light into understanding the role of LDH in the pathophysiology of other tick-borne pathogens.”
What is Lyme disease?
Lyme disease, or Lyme borreliosis, is a bacterial infection transmitted to humans through the bite of infected blacklegged ticks, often causing a distinctive rash, fever, fatigue, and headache, and potentially leading to complications if left untreated.

Cause: Lyme disease is caused by the bacterium Borrelia burgdorferi.
Transmission: The bacteria are transmitted to humans through the bite of an infected blacklegged tick (also known as a deer tick).
Tick Behavior: Ticks can become infected when they feed on infected animals, such as rodents, and then transmit the bacteria to humans when they bite them.
Geographic Distribution: Lyme disease is most common in the northeastern, mid-Atlantic, and upper Midwestern regions of the United States, but it can also occur in other parts of the country, as well as in Europe and Asia.
Symptoms:
Lyme disease symptoms can include a bull's-eye rash (erythema migrans), flu-like symptoms (fever, chills, fatigue, headache), and in later stages, neurological, heart, and joint problems.Early Symptoms (Stage 1):
Erythema Migrans Rash: A characteristic red, expanding rash that often appears at the site of the tick bite, sometimes with a bull's-eye appearance.
Flu-like Symptoms: Fever, chills, fatigue, headache, muscle and joint aches, and swollen lymph nodes are among the early symptoms of the disease.
Later Symptoms (Untreated Lyme Disease):
Neurological: Severe headaches and neck stiffness, facial palsy (Bell's palsy), meningitis (inflammation of the membranes surrounding the brain and spinal cord), numbness, pain, or weakness in the hands or feet are the common symptoms.
Cardiovascular: Heart palpitations or irregular heartbeat, and heart block (disruption of the heart's electrical system) are indicative of the disease.
Musculoskeletal: Arthritis (joint pain and swelling), pain, swelling, or stiffness in large joints, especially knees can also indicate Lyme disease.
Other: Dizziness or shortness of breath, inflammation of the brain and spinal cord, nerve pain, vision problems, and memory and concentration problems ("brain fog") are the other signs of the disease.
Treatment:
Lyme disease is treated primarily with antibiotics, with doxycycline and amoxicillin being common choices, and treatment duration varies depending on the stage and severity of the infection. However, if left untreated, Lyme disease can spread to joints, the heart, and the nervous system.Antibiotic Treatment:
Early Lyme Disease: A 10- to 14-day course of antibiotics is typically used for early Lyme disease.
Later Stages: Later stages of Lyme disease are also often treated with the same oral antibiotics, but for a longer duration (up to a month).
Intravenous Antibiotics: In some cases, especially for severe or complicated cases (like Lyme arthritis or neurologic disease), intravenous antibiotics (like ceftriaxone) may be necessary.
Common Antibiotics:
Doxycycline: Often used for adults and children over 8 years old.
Amoxicillin: A common choice for children under 8 years old and pregnant women.
Cefuroxime: Another option, especially for those who cannot take doxycycline or amoxicillin.
Azithromycin: Another option for patients unable to take doxycycline or beta-lactam antibiotics
Treatment Duration: The duration of antibiotic treatment depends on the stage and severity of the infection.
Localized skin infections: 14 days
Early disseminated infection: 21 days
Lyme disease-associated arthritis: 30 to 60 days
Lyme arthritis: 28 days
Post-Treatment Lyme Disease Syndrome (PTLDS): Some individuals may experience persistent symptoms (like pain, fatigue, or difficulty thinking) even after antibiotic treatment. This is called Post-Treatment Lyme Disease Syndrome (PTLDS). The cause of PTLDS is not fully understood, and there's no proven treatment for it. Continued antibiotic treatment hasn't been shown to improve symptoms or outcomes. Focus is on managing the symptoms with supportive care.
