Heart health: The confusing symptoms of heart failure you might be ignoring (including ‘bendopnea’)

We often think of heart disease as dramatic — sudden chest pain, collapse, or heart attack. But these days, it hardly is. Rather, it builds up bit by bit with silent symptoms that we tend to either overlook or ignore. Sometimes, even, there is another, quieter threat: heart failure. Its symptoms can be confusing, easy to dismiss, and even blend into everyday life. When ignored, these subtle signals might mean serious damage is already underway.

Furthermore, heart failure doesn’t even look the same for everyone all the time. Some people feel breathless, others feel tired all the time, or notice swelling. In some cases, small signs like nausea, confusion, or shortness of breath when bending forward (yes — even that is a real symptom!) may be the red flags. These might sound like harmless issues, but when clustered together, they can hint at a weakening heart.

In this guide, we’ll explore and explain how and why these signs arise, how the heart fails over time, and what to do next. Why? Because once you’re equipped with knowledge, you can act sooner, as the difference between a manageable disease and serious complications is basically time.

Bendopnea: Shortness of breath when bending over

This is a newer, less-known symptom. One recently described symptom is bendopnea: becoming breathless within about 30 seconds of bending forward at the waist. When someone with heart failure bends down (e.g., to tie their shoes or pick up something), fluid shifts and pressure on the heart and lungs increase, triggering difficulty in breathing. Because we often bend without thinking, many cases of bendopnea go unnoticed or are attributed to lung issues or simply being “out of breath.” Doctors note that bendopnea often appears when heart failure is advancing and the heart is under greater stress.

Orthopnea / Paroxysmal nocturnal dyspnea: Breathlessness when lying flat or at night

These are older, more well-known symptoms. Orthopnea means getting short of breath when lying flat (so many people sleep propped up on pillows). Paroxysmal nocturnal dyspnea (PND) is when someone awakens at night struggling for air, often after being asleep, needing to sit up to breathe better. These symptoms happen because lying flat shifts fluid back into the chest and lungs, triggering respiratory distress if the heart can’t accommodate the extra load.

Unexplained fatigue and activity limitation

You may feel unusually tired doing simple tasks like climbing stairs, walking, or even doing household chores. What was once easy now feels like effort. Because fatigue is so common in many conditions, patients often dismiss it. But when it’s new, worsening, or unexplained, especially alongside other signs, it’s a red flag.

Swelling (edema) and sudden weight gain

Swelling in ankles, feet, legs, or abdomen is a classic yet often under-noticed sign. This happens when the heart cannot push blood back efficiently, leading to fluid retention. Rapid weight gain (a few kilograms) in a short time may signal worsening heart failure.

Persistent cough, wheezing, or shortness of breath at night

Fluid buildup in the lungs (pulmonary congestion) can cause cough, wheeze, or a feeling of suffocation, especially when lying flat. You may wake up gasping for air (paroxysmal nocturnal dyspnea). Some cases even bring up frothy or blood-tinged sputum.

Digestive issues, reduced appetite, early fullness, nausea

Digestive symptoms like nausea, feeling full quickly, or loss of appetite sometimes accompany heart failure. This happens because blood flow to the digestive organs is impacted, and congestion in the abdomen presses on stomach structures. Fingers or toes may feel cold due to poor circulation.

Confusion, dizziness, palpitation, or light-headedness

Lower blood flow to the brain — especially when standing up — can produce dizziness, confusion, or faintness. Because these symptoms are common in many disorders, people may chalk them up to dehydration, low blood pressure, or simply aging. Besides, your heart may beat faster or irregularly (arrhythmia) to compensate for weakness. You might experience dizziness, lightheadedness, or even fainting (syncope).
