Why arm position matters

According to Dr Levine, proper arm placement is needed for the right blood pressure reading. The American Heart Association, says that one must always make sure the arm is supported, relaxed and at heart level, when taking a blood pressure reading. If the arm is not at heart level or is unsupported, gravity and muscle tension can increase, or decrease the pressure in the arteries, leading to misleading results. Arm position matters because the arm must be relaxed, supported, and at heart level to avoid gravity or muscle tension skewing blood pressure readings.

Overestimation: Arm too low or unsupported

When taking blood pressure measurements, it is crucial that the arm is properly positioned. If the arm is allowed to hang down at the side of the body or rests in your lap during this process, research indicates that the resulting blood pressure readings tend to be significantly elevated compared to when the arm is held in a position that is at the level of the heart.
A recent study conducted in 2024 and published in JAMA Network highlighted that when the arm is placed on the lap, the systolic blood pressure could be recorded as being 4 mm Hg higher. Underestimation: Arm too high

Conversely, when the arm is positioned above the level of the heart, there is a noticeable decrease in the pressure within the arteries. As a result, the blood pressure reading obtained will be inaccurately lower than the actual blood pressure. Such discrepancies can potentially result in misdiagnoses and may cause significant delays in receiving appropriate treatment.

Dr Levine says that arm position can alter the reading, regardless in any body position. Studies in emergency and ambulatory settings found that changing arm positions-parallel or perpendicular to the ground—can greatly affect the measured blood pressure, particularly when assessing for conditions like orthostatic hypotension. Several studies have shown that changing the arm position on purpose, can cause an interpretation of blood pressure changes, in situations.

Preventing misdiagnosis

Small changes in measurement technique can have big consequences. Overestimating blood pressure may lead to an incorrect diagnosis of hypertension and unnecessary medications, with their own side effects and costs.How to take your Blood Pressure at home

  • • Support the measured arm at heart level on a table or armrest. Keep the measured arm stable.
